Below you will find the new patient forms that will assist us in understanding you and your medical history more.
There are multiple formats for you to download. After downloading the packet (or individual pages), please fill them out and mail back to us at 818 N. Emporia, Ste 301, Wichita, Kansas, 67214. You may also email the completed forms to [email protected]. If your appointment is less than a week away, please hand carry the completed paperwork with you.
Please bring your insurance ID card as well as a photo ID. Co-pays are due at the time of the appointment. If you need assistance with a referral please let us know prior to your appointment. This is our photography/video consent form. We will take pictures, but it is up to you if you would like to share them to educate other patients on the plastic surgery process. Before and After pictures are a great tool to show realistic results. Make sure to look at Dr. Voge’s gallery and request to look at the book at your consultation.
